Required Items: Pre-portioned meals in labeled bags, medications in original packaging, flat or martingale collar, 6-foot leash, crate (optional), one safe chew toy.

Optional: Washable bedding if dog is not destructive.

Do NOT bring: bowls, plush or rope toys, rawhides, shock collars, or valuables.

By signing below, I acknowledge that I have reviewed the packing checklist and will follow the requirements.

I authorize MrwickK9 LLC to seek veterinary care for my dog in the event of illness, injury, or emergency during their stay.

I remain financially responsible for all veterinary costs. The trainer will make reasonable efforts to contact me first but may authorize treatment if I cannot be reached. I release MrwickK9 LLC from liability for veterinary outcomes.

I acknowledge that participation in training carries inherent risks, including illness, injury, escape, or death. I understand that dogs are animals with unpredictable behavior.

By signing below, I release and hold harmless MrwickK9 LLC its staff, and affiliates from any claims, damages, or losses related to my dog’s training, boarding, or transport. I remain legally and financially responsible for my dog at all times.

Scope of Services: MrwickK9 LLC agrees to provide Board & Train services including structured daily training, exposure to public environments, and progress tracking.

Owner Responsibilities: Provide accurate health and behavior history, food, medications, and approved items. Commit to follow-through at home.

Payment Policy: Full payment required before a dog can be picked up or released back to the owner or designee. Fees are non-refundable once training starts.

Cancellation Policy: Cancellations require 7 days’ notice for a partial refund minus deposit. Less than 7 days forfeits payment.

Progress Disclaimer: Results depend on owner follow-through. Trainer does not guarantee elimination of behaviors but will provide tools for improvement.

I grant permission for MrwickK9 LLC to photograph or video my dog during training for marketing, documentation, and educational purposes. I understand no compensation is due.
